Case Manager/Housing Locator (full-time) – Second Story for Homeless Youth

Second Story for Homeless Youth is a program providing young adults (ages 18-24) with support to find and maintain long-term housing, as well as counseling and life skills training. We are looking for an individual to work full-time (40 hours per week) as a Case Manager/Housing Locator. This individual should have at least two years of experience within the field of social services, preferably with the young adult population. Ideal candidates will be dependable, adaptable, compassionate, and culturally competent with strong communication, problem-solving, and conflict resolution skills.

Responsibilities:

  • Assist with locating low-cost housing for clients
  • Work with landlords to coordinate housing and troubleshoot client relationship issues
  • Assist with distributing donated items to clients
  • Conduct monthly life skills groups alongside other case managers
  • Provide on-call responsibilities for clients as needed
  • Visit with clients at least once per month and stay in touch through texts, emails, and phone calls
  • Transport clients and run errands within the community as needed
  • Attend weekly staff meetings
  • Work collaboratively with service providers in the community (e.g. medical professionals, other case managers)
  • Collaborate with county and private agencies to maintain and enhance existing services for clients
  • Provide case management and complete other documentation (e.g. statistics, assigned reports) in a timely manner

Schedule:

This is a full-time position working 40 hours per week. The schedule is somewhat flexible: mostly Monday through Friday for up to 8 hours per day between the hours of 8am-6pm with occasional evening/weekend hours as needed to support clients. Responsibilities can generally be split between virtual and in-person work.

Qualifications:

  • REQUIRED:
    • Bachelor’s degree, preferably in human services or a related field
    • At least two of relevant work experience, preferably with young adults
    • Ability to pass both criminal and child abuse/neglect background investigations
    • Valid driver’s license and evidence of insurability for obtaining auto insurance
    • A reliable personal vehicle to transport clients as necessary (NOTE: work-related mileage will be reimbursed at the current federal rate)
    • Computer literacy with Microsoft Office programs (e.g. Word, Excel, Outlook, PowerPoint)
  • PREFERRED:
    • Prior experience with case management
    • Spanish language fluency

Salary:

The salary range for this role is $50,000 to $53,000 per year depending on education, experience, and fluency in Spanish.
